Mobile Recovery Case Manager MA

Job Summary:

Job Description:
  • Provide intensive and mobile case management, care coordination and support and interventions to assigned consumers either in the community or at site based Respite.
  • Coordinate and case manage the day to day needs and overall service delivery of consumers at Respite or in the community as assigned by Clinical Supervisor or Program Director.
  • Support and assist consumers with day to day needs and overall service delivery providing direct support and community linkage to meet identified.
  • Coordinate and facilitate services which may include assisting consumers with managing income, insurance, other entitlements, accessing other services and community resources and scheduling/coordination of all medical and psychiatric.
  • Develop IAP’s (Individualized Action Plans) for each consumer as assigned. Ensure that each consumer is actively involved in their treatment planning process and their preferences are embraced.
  • Maintain progress notes and all documentation related to IAP’s and consumer contact. Ensure that documentation is written in a timely manner and is measurable, observable, objective and utilizes clinical terms
  • Develop and facilitate groups for consumers at the Respite program, as needed, that supports the varied needs of consumers (site and mobile) and support the skills necessary to maintain stability in the stability in the community.
  • Provide intensive supportive casework for assigned caseload including advocacy and assistance with access to entitlements to social, medical, psychiatric and community.

Outpatient Clinician (Bilingual preferred)

Job Summary:

Trinity care is seeking several strengths based/trauma informed Outpatient Therapists to provide evidence based counseling for children, teens, families, adults, and elders (therapists may work with multiple populations or specialize).

Job Description:
  • Provide crisis intervention, diagnostic evaluation, and treatment services utilizing a variety of treatment modalities such as individual, couple, family and group psychotherapy as clinically indicated.
  • Document service delivery to high professional standards in a timely and professional manner in the client record. Responsible for adhering to the clinic’s scheduling protocol, initiating outreach to clients for whom treatment has been interrupted, and for documenting all outreach attempts in the clinical record.
  • Establish effective working relationships with clients and utilizes solution based interventions which best address specific problem area as identified in the client’s treatment plan; relates well to diverse client population; displays good judgment; and in collaboration with clients, formulates and implements focused treatment plans that identify measurable goals and time frames for completion.
  • Submit all administrative and billing paperwork by the end of each business day.
  • Complete all clinical documentation, such as progress notes, diagnostic evaluations, treatment plans, quarterly reviews and termination/discharge summaries within the timeframes outlined in the agency’s policies, the content of which demonstrates clear evidence of medical necessity.

Fee For Service Outpatient

Therapeutic Training and Support

Job Summary:

Therapeutic Training and Support provide behavioral healthcare services to members of the community informed by up-to-date, bona fide research on factors that contribute to positive outcomes in psychotherapy.

Job Description:
  • Assist the In-Home Therapist in implementing the therapeutic objectives of the treatment plan to address the youth's social, behavioral, and emotional needs.
  • Assist in developing and supporting an effective risk assessment and safety plan with youth and family.
  • Assist youth and caregivers in building various coping, life, social, behavior, and emotion management skills.
  • Provide education and practice in emotional identification, behavior management, and coping skills coaching.
  • Model positive interactions with youth and family to enhance problem-solving, limit-setting, safety planning, communication, and skill building to strengthen family relationships.
  • Attend in-service training's as required.
  • Assist in developing initial and ongoing teamwork with family members, formal providers, natural supports, and others who can support sustainable progress.
  • Attend and/or travel to meetings/ appointments in various locations.
  • Maintain paper and electronic records in an orderly and timely manner.
  • Participate in weekly supervision.
  • If youth is enrolled in Intensive Care Coordination, participate in all care planning meetings and processes for the youth.
  • Follow through with on-call policies and procedures.
  • Attend agency orientation and all required trainings.

In-Home Therapy Clinician

Trinity Behavioral Health is seeking full-time mental health clinicians to provide home based services to families and youth under 21 years of age for our CBHI (Children Behavioral Health Initiative) program in the Metro Boston, MA area. We have full-time and part-time fee-for-service opportunities available.

Responsibilities and Duties Essential functions include:
providing age-appropriate, culturally sensitive In Home Therapy services for clients and families. Provide intensive family therapy; act as a liaison with collateral agencies and programs; identify and utilize community resources that support children’s and family’s growth; act as an advocate for families and help parents to build advocacy skills for themselves and their children. Act as part of CBHI wrap-around service model. Demonstrate cultural awareness and sensitivity.

Qualifications:
Master’s Degree in Social Work, Marriage and Family Therapy, Mental Health Counseling or related field. Ability to communicate effectively verbally and in writing. Licensed and insured driver with reliable and consistent access to a car. Evening availability required. Computer literate including Microsoft Office with the ability to learn new software applications. Bilingual in Spanish or Haitian Creole encouraged to apply.
